Annual Student Health Inventory

The Annual Student Health Inventory is required for each student entering or returning to the Loomis Union School District.  This form assist our staff in understanding any health related items for your student.  This form can be completed online in your Parent Portal Account using the Data Confirmation Process or by submitting a paper copy to your school office.
