Emergency Card

Annually an updated Emergency Card must be provided for each student in the Loomis Union School District.  This information can also be reviewed and updated in your Parent Portal account during the Data Confirmation process.  If you prefer to submit a paper copy of this form please print, complete, sign and return the form below to your school office.  You school office also has this form available in hard copy should you need assistance.
