Volunteer Personal Vehicle Use Permission Form

Students and chaperones attending district sponsored activities must ride the district provided transportation to and from each event. The use of personal vehicles for transporting students may only occur in the case of a unique circumstance and must be approved by the Superintendent. Individuals requesting to use their personal vehicle for transporting materials or students to school related activities must complete the form below and return it to our Business Services Department at 3290 Humphrey Road. Office hours are Monday through Friday from 7:30am-4:30pm.

Please plan ahead and allow 5-7 days for processing. Requests submitted without ample time for the approval process may be denied.
